If one’s asymmetry involves different amounts of breast projection or width from one side to the other, there are also options to use two different breast implant profiles from low to midrange to high. Fat grafting alone or in conjunction with two different size or profile implants may also be incorporated to improve symmetry. When asymmetry involves differences in shape or position of the nipple, it may be wise to choose smaller size breast implants, with a more conservative change in size so as not to accentuate the asymmetry. In those patients who have one nipple significantly lower than the other or one breast hanging or drooping more than the other, some type of lift for that particular side may be considered along with the breast augmentation. Careful planning before the initial surgery best helps to produce optimal results. It is also important to understand that complete healing and the final result after breast augmentation surgery truly takes several months – approaching one year – and what may be unsatisfactory to a patient after surgery may be something that will improve or resolve over time. Do not be too quick to insist that the surgeon correct a problem or concern when more time is recommended. To the touch, it has the feeling and consistency of a flexed muscle. Unlike breast implants, after this procedure is properly completed and healed, there is no reason to require additional surgery. These implants can be round or oval and come in a variety of sizes. A consultation and physical exam is needed to determine the appropriate size and shape for individuals. The butt implant procedure is performed in our state-of-the-art out patient facility in Miami under general anesthesia. Epidural (spinal) anesthesia may also be used at times. This surgical procedure takes approximately one and one-half to two hours to perform. Liposculpture and/or lifting procedures can add an additional one to three hours or longer. The surgical plan, including implant placement and liposculpture, is outlined prior to surgery. While in the sitting position, it must be assured that the implant is positioned above the weight-bearing area. After speaking with the anesthetist/anesthesiologist, the patient is brought to the operating room. After asleep, local nerve blocks, combined with a medication that shrinks blood vessels and capillaries to reduce any bleeding, are instilled. A 5 to 7 cm incision is made vertically in the midline between the upper aspect of the buttocks. A “pocket” or space is created under each gluteal muscle of the buttocks corresponding to the plan outlined on the skin. The implants are then inserted. Prior to the closure, a long acting anesthetic is poured around the implants, which dramatically reduces post-operative discomfort. The incision is closed with sutures below the level of the skin to avoid cross hatching and suture marks.

Liposuction is an approach to body contouring that removes excess subcutaneous fat through tiny incisions. Liposuction can be used alone or in combination with other procedures for cosmetic, posttraumatic, or reconstructive surgery. The distribution of fat is determined by three factors: heredity, metabolism and diet. Heredity determines the location and number of individual fat cells, which are fixed and unchanged after puberty. Metabolism and diet may increase or decrease the amount of fat in each cell, however it will not affect the number of fat cells or their distribution. At the Miami Center facility, located in Brickell, we use liposuction for the most precise removal of fat deposits, because it is the only proven way to reduce fat in localized areas of the body. There is no way to “spot” reduce or selectively diet away specific areas of localized fat. With liposuction, the actual fat cells are removed, making the procedure permanent. As a result the body will always have less fat in the treated areas. If weight is gained following this procedure, the remaining fat cells will enlarge along with the other fat cells throughout the body, however, your body will retain these improved proportions. Liposuction is suitable for patients with minimal to moderate fat accumulation that is not correctable by restrictive weight loss, dieting, and exercise. Liposuction is not a substitute for weight loss, although it has been reported, perhaps anecdotally, to have the beneficial side effect of stimulating patients achieve better weight control. Liposuction is not successful in treating generalized obesity. Patients who are generally overweight, but who exhibit unsightly bulges of fat, are also candidates for treatment of these areas. Because liposuction does not reduce the amount of skin present, the best results are obtained in those patients with good skin tone/elasticity because their skin readily contracts to the newly contoured frame. Pleasing and satisfactory results can also be obtained in older patients with somewhat less skin elasticity. Those patients with more skin laxity, or poor elasticity may benefit from or require removal of the excess skin to achieve intended results, as would be the case in an abdomen with hanging loose skin or abdominal “apron” In Miami, liposuction is now nearly as common for men as for women. Liposuction Surgery in Florida Through tiny incisions, no more than the width of a pencil eraser, a tiny stainless steel tube or “cannula” is used to sculpt the fat in treated areas. The small cannula reduce trauma and allow greater precision and control. About the Tummy Tuck Surgery Every tummy tuck performed at our Miami, Florida cosmetic surgery center is performed with the utmost care and latest techniques. Abdominoplasty, or tummy tuck surgery generally takes between two and three, hours plus additional time for liposculpture, if performed. The line of incision and areas for liposculpture are outlined. After speaking with the anesthetist or anesthesiologist, the patient is brought to the operating room. Once asleep, medication is administered to reduce bleeding. Tummy Tuck surgery also results in a scar around the belly button (umbilicus), as this is made free to allow removal of the excess and loose skin. During a tummy tuck, skin and fat are separated from the abdominal wall, by a transverse incision across the lower abdomen to the lower portion of the rib cage. The elevated skin is then lowered and the excess removed. The belly button (umbilicus) is then repositioned to its normal location. Prior to closure, a long-acting anesthetic is administered along the muscle wall and incision, which dramatically reduces post-operative discomfort. The abdominoplasty incision is closed with sutures predominantly below the level of the skin to avoid cross-hatching and suture marks. A light dressing and elastic binder is then applied.

Facelift surgery can bring youth back to a patient’s face. In youth, the face is firm and smooth due to good skin elasticity and proper positioning and volume of normal facial fat. A face and neck lift procedure works to restore the “triangle of youth” Generally, as can easily be seen in any fashion magazine, a youthful face ideally has high cheekbones, a full cheek and defined jawline. These three attributes shape what is commonly referred to as the “triangle of youth” with the base of the triangle aligning with the full cheekbones and the point at the chin. As we age, this youthful triangle gradually becomes inverted. It is now well understood that facial aging, in simple terms, is due to a combination of skin’s loss of collagen and elasticity as well as facial volume loss of bone and fat. Studies have shown that multiple areas of facial skeleton undergo bone resorption and changes in shape as we age. Studies have also revealed that the face has multiple deep fat compartments. These fat compartments, generally full in youth, diminish in volume over time. As facial bone and fat volume diminishes with age, the overlying skin, no longer full and supported, becomes hollow, loose and sags. Analogy can be made of the change in appearance from a grape to a raisin. As the internal volume of a grape diminishes and shrinks, the once firm and plump grape skin shrinks and wrinkles. Volume restoration, especially fat volume restoration is often a very important step in facial rejuvenation. The idea is to restore that “facial triangle of youth”. Depending on each individuals exam, volume may be restored to hollow and volume depleted areas of the temples, orbits of the eyes, around and under the eyes, the midface, cheeks and lips in conjunction with the facelift procedure. This volume restoration is most often accomplished with one’s own fat. A facelift will not, however, completely remove every fold, line, and wrinkle. There is limited improvement around the eyes and with fine lines of the facial skin. A facelift alone will not improve “smoker’s” lines around the mouth or a poor and damaged skin complexion. Other options exist to treat these problems, including CO2 Smart Skin Fractional Laser resurfacing treatments, dermabrasion, and chemical peels. Frequently, chin implants are placed to improve the profile proportions of the jaw line and increase the projection of the chin. Facelift Techniques It is important to realize that there are numerous facelift techniques. These range from superficial skin re-draping and fat contouring procedures to more extensive procedures of tightening of the deeper connective tissue layer of the muscles known as the SMAS ( superficial muscular aponeurotic system ) along with tightening the muscles in the neck called a platysmaplasty ( repairing the pair of platysma muscles of the neck ). With all of these techniques, the facial skin is elevated, to varying degrees, and the excess removed. Understand that a youthful face is not created by simply elevating and pulling on skin. Correcting and shaping the underlying foundation of muscles, connective tissues and fat compartments to recreate that “triangle of youth” is the key component before removing the excess skin and closing without tension.

As we grow older, the neck is one of the first places where we tend to show our age. Many people feel they develop fatty jowls as they age. This is sometimes referred to as a “turkey wattle” and is caused by creasing under the neck. This can make us look older than we feel. With a neck lift, the appearance of the neck is enhanced, because the skin and neck muscles are tightened. This works to improve the contour of the jaw line as well as the appearance of the neck itself. Often, a neck lift is performed in conjunction with liposuction or a facelift, but it can be performed on its own as well. Neck Lift Consultation During a neck lift consultation, Dr. G will discuss the goals of the surgery as well as rejuvenation options. He will also ask about any medical conditions or drug allergies you have, current medications you are on, and any past medical treatments you have undergone. Dr. G will also evaluate your general health status and make sure you are healthy enough for the procedure. The surgeon will examine and measure your face and take “before” photos and then discuss his recommended course of treatment. During the consultation, he will discuss the likely outcomes, the anesthesia that will be used during surgery and any risks or possible complications. During the consultation, be sure to ask any questions you may have. Neck Lift Surgery The procedure generally takes two to three hours. During surgery, small incisions are made behind the ears and under the chin. Through those incisions, excess fat will be removed, usually through liposuction, but it can be done through surgical excision as well. The neck muscles are then tightened, and sometimes a small section of the platysma muscle is removed. After this has been completed, the skin is replaced and is pulled taut. The incisions are then sutured closed. Any excess skin will be removed. Recovery After Neck Lift Surgery Dr. G will provide you with detailed information for your post-surgical care, and he will tell you when a follow-up appointment should be scheduled. Right after the procedure, your neck will be bruised and it will feel tight and numb. The bruising will go away in four or five days, but the tight and numb feeling may remain for several months. After surgery, you will need to keep your head elevated above your heart in order to minimize swelling. Make sure not to twist or bend the neck excessively, and do not apply ice to the bruising, as this can compromise blood flow. The amount of time needed for recovery varies from person to person. Potential Neck Lift Complications Possible, but unlikely, complications and risks associated with neck lifts include bleeding, temporary hair loss along the incisions, facial asymmetry, a rare nerve injury that could cause weakness in your lower lip, hematoma, scarring, or infection. There is a low risk of deep venous thrombosis.

Dysport® and BOTOX® Cosmetic Treatments Vs. a Brow Lift One of the first parts of the face to begin showing signs of aging is the forehead. Initially, glabellar creases (vertical wrinkles) develop between the eyebrows, followed by horizontal wrinkles across the forehead. At the onset of these forehead lines and wrinkles, neuromuscular paralyzing agents, such as BOTOX® Cosmetic and Dysport® can be used; however, neither of these treatments is permanent. Furthermore, these paralyzing agents cannot address the wrinkling that occurs over the bridge of the nose. Additionally, as an individual continues to age, the brow itself begins to descend. When the descent is mild, Dysport® and BOTOX® Cosmetic can be used to treat forehead wrinkling and lift the eyebrow; however, these products are just a temporary fix. At some point, the results achieved with Botox may become difficult to see: This is when a brow lift should be considered. What Causes the Forehead to Show Signs of Aging? Gravity, the continuous use of the forehead muscles, smoking, genetic influences, sun exposure, fluctuations in weight and loss of skin, and muscle tone all contribute to the changes in the elevation of the brow. Nasal surgery, or rhinoplasty, is popular among men and women who are dissatisfied with the size and/or shape of their nose. Being such a prominent feature, the nose may be the most defining characteristic of one’s face. Septorhinoplasty corrects both cosmetic concerns and septal deformities causing airway obstruction. Septoplasty, alone, may be performed to correct a deviated septum causing airway obstruction. Chin augmentation or a chin implant may be considered and discussed at the time of the nasal surgery consultation as this procedure is frequently performed at the same time as rhinoplasty to improve and balance facial proportions. Cosmetic improvements and correction of deformities of the nose can be corrected by adding to, removing, reshaping and/or rearranging bone and cartilage. Each person’s nose is different requiring a complete understanding of nasal anatomy, along with a full armamentarium of procedures and techniques to formulate a specific surgical plan. Bone and cartilage can be filed, shaved, cut and trimmed along with a vast number of suturing techniques to reshape cartilages of the nostrils and tip. More often, precise cartilage grafts are trimmed and shaped to provide added contour and support to the dorsum, tip and nostrils for cosmetic as well as functional repairs. Cartilage for repairs are most often harvested from the nasal septum, however, can also be taken from ear and rib cartilage as well. At times, soft tissue fascia and/or dermal-fat grafts can be used as well for corrections along the dorsum. On occasion, allograft donor dermis and cartilage can be used as well. Techniques Dr. Gershenbaum routinely performs both closed rhinoplasty and open rhinoplasty procedures depending on the needs of the patient. Rhinoplasty and septorhinoplasty can be accomplished through a closed technique, where incisions are made inside the nose only. Dr. Gershenbaum performs closed nasal surgery with more minor rhinoplasty procedures, such as removing a nasal hump, narrowing the nasal bridge or adding simple cartilage grafts to support the nostrils or add conservative projection to the tip. For more extensive rhinoplasty and septorhinoplasty procedures, Dr. Gershenbaum prefers an open approach, in which an incision is also made across the columella (the skin bridge between the nostrils), which connects the internal incisions. The open approach provides unparalleled exposure and accuracy with greater predictability in results. The performance of each suture technique, cartilage shaping and graft placement is visualized. The resultant scar is near imperceptible. Dr. Gershenbaum has never had an issue or complaint about the few millimeter columellar scar in all his years of practice.

BOTOX®, or botulinum toxin, is a nerve impulse “blocker.” It binds to the nerve endings and prevents binding of the chemical transmitters that activate the muscles. BOTOX® Cosmetic usually take full effect within one to two weeks. Treatment will lasts from four to six months, and results may vary for duration and degree of improvement. At times, repeat injections between two and three weeks may be necessary.
