  Drain-free abdominoplasty with pubic lift and liposuction.Abdominoplasty, commonly known as a tummy tuck, is a popular cosmetic procedure that helps flatten the belly and tighten the abdominal area. This surgical procedure is especially beneficial for patients who have loose skin or excess fat due to factors such as pregnancy, aging, or significant weight loss. There are many reasons why patients choose abdominoplasty. A group of patients we see more commonly now are those who have been taking Semaglutide medications such as Ozempic® or Wegovy®. These are a prescription medication used to treat type 2 diabetes and to promote weight loss. Many patients who take Ozempic® have reported significant weight loss results. In some patients a scar-free skin tightening procedure such as Morpheus body may be enough. However, even moderate weight loss, often results in excess skin. In such cases, an abdominoplasty, also known as a tummy tuck, may be needed to achieve a smoother and firmer abdomen. What is Abdominoplasty (Tummy Tuck)? Abdominoplasty is a surgical procedure that involves removing excess skin and fat from the abdomen and tightening the abdominal muscles (when needed). The result is a firmer, flatter, and more contoured abdomen. The procedure can be customized to meet the needs of each individual patient, ranging from a mini tummy tuck to a full tummy tuck. With her background in gender affirmation surgery, Dr Gallagher understands the different needs a patient may have depending on their gender. For example, whereas female patients often seek a sinched waste a male patient will often want a more athletic look. We therefore are very careful to design the procedure keeping each individual’s preferences in mind. Depending on the patient’s needs Dr. Gallagher often adds in liposuction as part of the sculpting technique. What is special about the Drain-free Technique? One of the most significant advancements in tummy tuck surgery is the development of a drain-free technique which Dr. Gallagher has published on. Traditionally, abdominoplasty procedures required the use of drains, which were tubes inserted into the incision site to help remove excess fluid that accumulates during the healing process. However, Dr. Gallagher specializes in a drain-free technique utilizing a special stitching technique that offers several advantages. Faster Recovery Time One of the most significant advantages of drain-free abdominoplasty is the faster recovery time. Traditional tummy tucks require patients to have drains for several days, which can be uncomfortable and limit mobility. With the drain-free technique, patients can avoid the discomfort of having drains, which allows them to recover more quickly and resume their normal activities sooner. Less Discomfort Having drains inserted during surgery can be uncomfortable and cause additional discomfort during the recovery process. Patients who undergo drain-free abdominoplasty experience less discomfort because they do not have to deal with the discomfort and inconvenience of drains. Additionally, they do not have to be removed while the patient is awake. What about the pubic area? At the same time as abdominoplasty Dr. Gallagher will also address the pubic area. Depending on the patient’s anatomy this can also be lifted and reduced at the same time as abdominoplasty. What to Expect from Tummy Tuck Before undergoing an abdominoplasty, patients will need to undergo a consultation with Dr. Gallagher to determine if they are good candidates for the procedure. Patients should be in good overall health, have a stable weight, and have realistic expectations about the results of the surgery. Abdominoplasty is a surgical procedure that requires general anesthesia. The procedure can take between 2-4 hours, depending on the extent of the surgery. During the procedure, Dr. Gallagher will make an incision in the lower abdomen, remove excess skin and fat, and tighten the abdominal muscles. The incision will be closed with dissolvable sutures, and patients will be given instructions on how to care for their incision site and manage any discomfort. Dr. Gallagher’s drain-free technique eliminates the need for drainage tubes, and all stitches are dissolvable, allowing patients to shower just a few days after surgery. Recovery time varies, but most patients need between three and six weeks off work and exercise.

Liposuction Benefits One of the biggest advantages of liposuction is that it permanently removes fat cells from the targeted area. Once these fat cells are removed, they will not regenerate, and the area will remain slimmer and smoother. This means that if a patient gains weight after liposuction, the fat may distribute to other parts of the body instead of returning to the treated area. It is therefore the most powerful way to reshape the body whether its sinching in a waste of getting rid of unwanted curves and love handles for a more athletic result. If the sagging skin is mild, she may recommend a non-invasive skin tightening procedure such as Bodytite or Morpheus body in addition to liposuction. This way skin can be tightened without scars. What is liposuction is REALLY effective for? The two happiest groups of liposuction patients we have are patients with annoying love handles. While these areas may shrink with diet and exercise it can be really difficult to get rid of them. This procedure will do just that and the skin at the sides is relatively forgiving and usually shrinks down well to the new more athletic shape. The second group of patients are folks with a “double chin”. Lipo suction can get rid of this in a few minutes. Many of my patients add this on when they are having another procedure. Procedure Details Liposuction surgery is performed under general anesthesia, and the duration of the procedure varies depending on the extent of the treatment. Patients can go home the same day, and recovery time varies from one to three weeks depending on the extent of the procedure. It is recommended that patients wear a compression garment for six weeks after surgery to prevent swelling and optimize results. It takes many months to see the final results. • Breast Augmentation

  Breast augmentation still remains one of the most in-demand and gratifying procedures Dr. Gallagher performs. Dr. Gallagher draws on her experience as a breast reconstructive and gender affirmation surgeon to deliver customized results to her patients. She prides herself on listening to, and carefully planning with patients to reach their goals. Most patients will request a natural full result, which is Dr. Gallagher’s specialty. Dr. Gallagher’s technique uses the implant to push forward and showcase the natural breast so the “operated look” is avoided. The key to this is choosing the correct implant size for the patient’s body. In this way recovery is fast and the patient gets a long-lasting beautiful result with none of the telltale signs of a breast augmentation. Implant Size Dr. Gallagher will spend a lot of time deciding on size with a patient before surgery. While this is ultimately up to the patient, there is certainly a sweet spot when it comes to size. Every woman has enough space for a particular implant size- if a patient sticks to that size there will be many benefits. The result will be very natural, will last longer,and because no muscle cutting or stretching is required recovery will usually be much faster and easier. Breast Augmentation Technique Incision There are three main routes through which a breast implant may be positioned. Incision choice is important, because when you have a lovely natural breast augmentation, often the only way to possibly tell a patient has had surgery, is the incision. Dr. Gallagher specializes in keeping that incision small, and placed in the best possible position for healing. The three main options for incisions are: Through the infra-mammary fold. This is normally the choice of the majority of Dr. Gallagher’s patients. This is because the incision can be kept quite small depending on the size of the implant, and also because it heals extremely well, therefore in most patients it will be almost invisible. When we look at the chest, it is hidden from view in the fold, and the slight discoloration will fade to match the skin color of the fold in about a year. This incision preserves the ability to breast feed, and also preserves sensation to the nipple. Through the areola or the edge of the nipple itself. Through the armpit or trans-axillary approach. This can usually only be done after silicone implants for breast augmentation. Implant Position The next question to be considered is where to position the implants? Again there are a number of options for this, implants can go on top of, or underneath the muscle, or a combination of these, where the implant is half covered by the muscle and half covered by the breast. Dr. Gallagher offers all three as part of her tailored approach for patients. Let’s start with the most common position patients choose, which is underneath the muscle. Underneath the Muscle Dr. Gallagher will most commonly choose this position, designing a pocket underneath the muscle, with minimal blood loss, so that the patient’s recovery is fast. This position is advantageous because, at the upper part of the breast there is a smooth transition, as the upper part of the implant is hidden underneath the muscle, thus avoiding the telltale rounded out appearance at the top of the implant. This is the choice of most girls who want beautiful results showcasing the natural breast. For most women underneath the muscle is best, because the rate of capsular contractures or hardening of the implant is lessened if it can be positioned here. Underneath the breast itself Alternatively, for some of our patients, particularly our transgender women, we may choose to put the implant underneath the gland itself, or underneath the breast itself. One instance, in which we would choose to do this, is in what we call a tuberous breast, where the shape of the breast is not very favorable to begin with, or when the fold is very high, and we don’t have a rounded out bottom to the breast. Often it is best in that situation to rely on the roundness of the implant itself to provide that, and so in order to make sure that roundness can be readily seen, we put the implant underneath the gland itself. Dual Plane Using a dual plane technique, Dr. Gallagher might position the top part of the implant under the muscle, and the bottom part under the breast. With this technique a little lift effect can be created. Dr. Gallagher will do some dissection up underneath between the implant and the breast tissue, allowing the breast to slide upwards, a little ,in order to get the illusion of a raised nipple or very subtle lift. This is useful in patients with some sagging.

  Breast lift surgery, also known as mastopexy, is a cosmetic procedure that has become increasingly popular among patients who have experienced sagging breasts due to various factors such as weight loss, age, genetics, or breastfeeding. The aim of this surgery is to restore the shape, size, and position of the breasts and improve their overall appearance, which can have a significant impact on a woman’s self-esteem and confidence. Weight loss and Breasts- what are “Ozempic® Breasts”? Many individuals are turning to weight loss programs including powerful new drugs such as Ozempic® or Wegovy® to help them achieve a healthier and more active lifestyle. However, one common side effect of significant weight loss is the loss of breast volume and elasticity, which can lead to sagging or drooping breasts. This is a common concern for many Ozempic® weight loss patients, who sometimes use the term “Ozempic® breasts” as it can affect their appearance and self-confidence. When a person gains weight, the breasts can also increase in size due to the accumulation of fat in the breast tissue. However, when a person loses weight, the breasts can lose volume, and the skin may not have enough elasticity to retract, causing the breasts to sag. This can be especially noticeable in patients who have lost a significant amount of weight quickly (which we commonly see with new weight loss treatments) as the skin may not have had enough time to adjust. A breast lift, also known as a mastopexy, can help to address this issue by lifting and reshaping the breasts to restore a more youthful and perky appearance. What is involved in a mastopexy? During a mastopexy procedure, Dr. Gallagher will make incisions on the breasts to remove excess skin and tissue. Often, an anchor-shaped scar around the nipple and base of the breast is needed. The nipple and areola are then repositioned to a higher position on the breast, and the remaining tissue is reshaped and lifted to create a more youthful and perky appearance. Typically, we “overlift” the breasts as they will settle into a very natural and perky appearance in the weeks following surgery. In some cases, breast implants or the patient’s own fat taken from elsewhere on the body may also be used during the surgery to restore lost volume and improve the overall shape and size of the breasts. The plastic surgeon will customize the approach based on the individual patient’s needs, size goals, and body shape to ensure the best possible outcome. Dr. Gallagher is an experienced plastic surgeon who offers customized mastopexy procedures tailored to each patient’s unique needs and goals. She uses advanced surgical techniques and the latest technology to provide patients with safe and effective results. Breast Lift Recovery After surgery, patients may experience some swelling, bruising, and discomfort, which can be managed with pain medication and proper post-operative care. Most patients can return to work and resume normal activities within two weeks of the procedure. Typically, our out-of-town patients stay for about a week. Patients will go under general anesthesia for this procedure and go home the same day. Usually, they will return to work about two weeks after the surgery. All stitches are dissolvable and again Dr Gallagher never uses drains for this procedure. What about the scars after breast lift? One of the most common concerns among patients who are considering mastopexy is whether their scars will go away after the procedure. While scars are an inevitable part of any surgical procedure, there are several factors that can impact the appearance of scars after mastopexy. The location and size of the incisions made during mastopexy will depend on the individual patient’s needs and goals, as well as the severity of their breast sagging. In general, the incisions will be made around the areola and down the breast, creating a lollipop or anchor shape. While these incisions may result in some visible scarring, the good news is that the scars can usually be concealed by clothing or a bra. It’s important to remember that every patient heals differently, and the appearance of scars can vary from person to person. Some patients may experience thicker, raised scars, while others may develop lighter, flatter scars. Additionally, some patients may be more prone to developing keloid scars, which can be more difficult to treat. Scar Prevention Care However, there are several things that patients can do to help minimize the appearance of scars after mastopexy. Dr. Gallagher will instruct patients on their initialized scar care regimen. While it’s impossible to guarantee that scars will completely disappear after mastopexy, many patients find that their scars become less noticeable over time. In fact, scars can continue to improve for up to two years after surgery.

• Breast Reduction

  Large breasts can cause a host of problems for our patients such as, neck, back and shoulder pain. In addition finding suitably fitting clothes is often problematic (forget about button-down shirts) and rashes underneath the fold can be annoying. Heavy large breasts usually sag sooner or later, and many of our patients abhor their shape as well as their size because of the frequent lateral overhang of fat. Breast reduction decreases the size of the breasts but in addition Dr. Gallagher always lifts and shapes the breast as part of the procedure. Why Breast Reduction? Usually large breasted patients have tried a number of other options such as weight loss before seeking surgery. Unfortunately genetics are usually at play and a patient with “macromastia” (the medical term for large breasts) will rarely be able to reduce the breasts significantly or find ways of easing the back pain short of the reduction surgery. What to Expect? At your visit Dr. Gallagher will talk to you about your general health and wellness before doing a very important exam of the breasts (safety is paramount!). Dr. Gallagher will then plan the breast size with you and discuss which incisions will be needed to reduce the size and lift the breasts. Breast Reduction Surgery This surgery is done under general anesthesia with some patients choosing to stay overnight. No stitches need to be removed and a patient can take a shower in about two days. Typically a patient will return to work about two weeks after the surgery. Dr. Gallagher never uses drains for this procedure.

• Injectables

  All injections are done by Dr. Gallagher. Botox Botox is a simple temporary injection used on the face to prevent and improve the appearance of lines and wrinkles. Dr. Gallagher will use an individualized approach to identify and treat the exact spots causing the lines – this way giving much more “bang for your buck” than a cookie-cutter approach. Dermal Fillers As the face ages it loses volume. Dr. Gallagher uses a variety of different techniques to restore this volume and correct signs of aging. In this way a patient can have a very effective “liquid face-lift” with not much down-time. As well as reversing the signs of ageing, different fillers can also be used to enhance a patient’s natural features.
